Terrible customer service all the way up the chain. Do not use this form unless you want your form to stop working randomly and you want to be triple charged without realizing it. Oh and customer service will string you along over a month without a proper resolution. They will offer a fraction of the refund and tell you not to email all of the support agents who never responded. STAY AWAY!

BoloForms has concluded its assessment of your affiliate account and related commissions. As a result of this review, your participation in the BoloForms affiliate program has been permanently terminated, effective immediately. This decision is final.

During our evaluation, we identified significant inconsistencies between reported conversions and the outcomes we independently observed across the same intent-based channels. While we previously issued a goodwill payment of USD 1,000, this payment should not be interpreted as ongoing validation of subsequent activity. Following deeper analysis, we determined that additional commissions cannot be approved or released.

As outlined in our affiliate program terms, BoloForms reserves the right to withhold, reverse, or void commissions and to terminate affiliate relationships in cases where traffic quality, attribution integrity, or promotional practices raise material concerns, even if such practices are not immediately evident at the time of earlier payouts. We are not obligated to disclose internal detection methodologies, investigative processes, or platform-specific attribution safeguards as part of this review.

No further commissions are payable.
No additional reviews or escalations will be conducted.
Any future participation in affiliate, referral, or promotional programs associated with BoloForms or its related products is not permitted.
We consider this matter closed and request that no further correspondence be sent regarding affiliate commissions or reinstatement. Any continued outreach on this topic may not receive a response.
